Hello, my name is Veronica Dietrich. I am a resident physician with the Department of Emergency Medicine at the University of Missouri Kansas City and University Health (Truman Medical Center). I am conducting a research study about the effect of outdated regulations on information transfer between nursing homes and emergency departments. To answer this question, I would like the opinions from the Emergency Medicine Physician Community in the state of Missouri. The survey will address questions related to: Does the Missouri ACEP population think that the current nursing home information transfer is inadequate and what information should be included in the transfer documents? Ultimately the goal would be to develop a consensus list to be used to develop policy regarding information that would ideally be required for a transfer?

If you choose to participate in this study you will be asked to complete a brief survey that would take about 10 minutes of your time. 

Your participation is entirely voluntary; you may skip any questions that you don’t want to answer or choose to stop participating at any time. 

No personally identifying information is being collected. You will not be identified in any reports about this research. Your email address CANNOT be linked to your response.
